ReNU syndrome is a rare genetic neurodevelopmental disorder caused by variants in the RNU4-2 gene, characterized by developmental delay, intellectual disability, and microcephaly. First identified in 2024, it affects brain function, growth, and physical coordination.
Symptoms
Key features include moderate-to-severe developmental delay, low muscle tone (hypotonia), and seizures.
Causes
ReNU syndrome is caused by specific genetic changes, specifically dominant variants in the non-coding RNA gene RNU4-2. These errors disrupt the normal work of the body’s spliceosome, which manages cell instructions.
- Gene Mutation: Changes happen in a tiny, critical region of the RNU4-2 gene located on chromosome 12.
- Spliceosome Disruption: The gene makes U4 small nuclear RNA, a key part of the major spliceosome needed for proper RNA processing.
- Autosomal Dominant: Most instances occur as de novo (new) random changes in the affected individual rather than being passed down from parents.
- Inheritance Exceptions: Very rarely, a mild parent can pass the altered gene to a child.
Prevention
There is no known way to prevent ReNU syndrome because it is a genetic neurodevelopmental disorder caused by de novo (spontaneous) mutations in the RNU4-2 gene. Because these mutations typically occur randomly during early development rather than being inherited from parents, primary prevention is not possible
Diagnosis
ReNU syndrome diagnosis is confirmed via targeted genetic testing or whole-genome sequencing to identify heterozygous, de novo pathogenic variants in the non-coding RNU4-2 gene (most commonly the recurrent n.64_65insT insertion). Clinical evaluation typically involves assessing distinct neurodevelopmental, facial, and structural features.
Prognosis
The prognosis for ReNU Syndrome (associated with RNU4-2 variants) involves lifelong global developmental delay and intellectual disability, though survival into adulthood is documented. While formal statistics on reduced life expectancy remain unestablished, diagnosed adults have been reported in their late thirties and mid-to-late forties.
- Intellectual Disability: Around 65% of individuals experience severe impairment, 30% moderate, and 5% mild or borderline.
- Speech and Communication: Most individuals are non-verbal or have severely limited speech, though many successfully learn alternative communication methods.
- Mobility: Motor delays are common; while many learn to walk (often with an ataxic or unsteady gait), others remain non-ambulatory or require mobility aids
Treatment
There is currently no cure or FDA-approved disease-modifying medicine for ReNU syndrome, but management relies on multidisciplinary supportive therapies, symptom-specific interventions, and emerging investigational research.
- Physical therapy (PT): Strength training, balance, coordination, and mobility support.
- Occupational therapy (OT): Managing motor skill delays and sensory integration.
- Speech-language pathology (SLP): Improving communication, utilizing AAC devices, and addressing swallowing difficulties (dysphagia).
- Feeding therapy: Nutritional support, calorie-dense formulas, or gastrostomy tube placement for severe failure to thrive.
- Neurological care: Standardized anti-seizure medications (ASMs) administered by a neurologist for epilepsy.
- Gastrointestinal care: Laxatives, softeners, or prokinetics for constipation, and positioning or anti-reflux medications for GERD.
- Orthopedic and bone health: Monitoring and treatment for low bone density, hip dysplasia, or scoliosis using physical medicine, calcium/vitamin D optimization, or bisphosphonates.
- Sleep and behavior: Melatonin or specialized support for sleep disturbances.
- Antisense Oligonucleotide (ASO) Therapy: Early-stage research is underway through collaborations like
BioMarin and n-Lorem Foundation to develop targeted ASO candidates designed to address the underlying RNU4-2 genetic splicing defect.
